Every Mind Primary Care is a nurse practitioner-led primary care practice dedicated to providing compassionate, person-centered healthcare for adolescents, adults, and older adults in the comfort of their own homes. As a teaching site affiliated with Boston Medical Center and University of Massachusetts Boston, the practice supports the education of future healthcare professionals while delivering high-quality clinical care.
Every Mind Primary Care specializes in caring for individuals experiencing cognitive, communication, sensory, memory-related challenges, and other neurocognitive conditions. Through a combination of home-based primary care and telemedicine support, the practice provides accessible, individualized healthcare tailored to each patient's unique needs and circumstances. Services include preventive care, chronic disease management, care coordination, and Applied Behavior Analysis (ABA) services designed to promote health, independence, and quality of life.

Dr. Jessica Bardina
Jessica Bardina, DNP, AGNP is a board-certified Adult-Gerontology Nurse Practitioner, educator, and healthcare leader with expertise in primary care, population health, and nursing education. She serves as faculty at the University of Massachusetts Boston and is affiliated with Boston Medical Center, where she contributes to the education and clinical training of future nurse practitioners.
Dr. Bardina is the founder of Every Mind Primary Care, a nurse practitioner-led practice dedicated to delivering home-based primary care and telemedicine services for individuals experiencing cognitive, communication, sensory, memory-related challenges, and other neurocognitive conditions. Her clinical interests include improving access to care for underserved populations, supporting caregivers and families, and developing innovative models of integrated primary care.
In addition to her clinical and academic work, Dr. Bardina is actively involved in research, workforce development, and healthcare innovation focused on expanding access to high-quality, patient-centered care.
